I'm trying to use proximity sensing  in PSOC 4100S Plus to sense hands within a specified distance of 5 cm.

To make this, I made the sensor by the PCB routing that you recommended, and It's sensor size is 5 cm length & 2 mm width. 

I tried every combinations of the filters (except ALP), and found that it's hard to achieve the specified distance.

Maybe the ALP filter is our solution, but it doesn't seem to be able to use now, or do you have any alternate solutions ?

Are you referring to the "https://www.cypress.com/documentation/application-notes/an92239-proximity-sensing-capsense" design and tune your sensor?  5cm distance is very challenged for a wire sensor(5 cm length & 2 mm width) .

As mentioned in the above application note,  Factors Affecting Proximity Distance are:

Proximity-sensing distance depends on the following hardware, software, and system parameters:
• Hardware parameters
 Type of the sensor
 Size of the sensor
 Parasitic Capacitance (CP) of the sensor
 Overlay material and thickness
 Nearby floating or grounded conductive objects
 Software parameters
 Resolution of the sensor
 Firmware filters
 System parameters
 Power consumption
 Response time
 EMI/EMC/ESD performance

Could you share us your PCB file? 

It is not very clear to understand what you say about the sense size 5cm? Circle? Rectangle? Or other shape?

Now, what does the distance it can reach?

Sorry for the late reply, our customer reduced the requirement for sensing distance to 3 cm.

This distance is ok for our board, so we do not have any problems any more.
