Hi,

The subject MOSFET PSPICE model is a SPICE3 Model with thermal RC Network, but I don't see where the TCase and Tj terminals are defined in the model.

 

I pasted the model below.  Refer to the attachment, section 3 where it mentions that there are five connections for the thermal model.

I can see that there are only 3 nodes in your simulation model. 

To derive the level 3 model from the simetrix please follow the steps mentioned in the below thread,

For additional details you can also refer the user manual,

Please let me know if you were able to create L3 model.
