A sporadic PLL loss of lock is seen. What could be a reason?

Check the Errata Sheet of the specific device. For some devices special care needs to be taken when initializing the PLL (workarounds).
This is the most likely reason. Others would be wrong configuration (overclock, any case violating data sheet parameters for operating conditions).
